School Enrollment Form:  Can be used instead of three proofs of residency during the regular season and All-Stars. Must be printed and provided to Board member at either sign ups or try outs after signed by school principal.

Volunteer Form:  MUST be printed, signed, and provided to the President at either sign ups or try outs. Background checks must be performed on all volunteers who are coaching, umpiring, or assisting during practices and games.

Safety Plan: Coaches should read the safety plan and then sign-off on a copy in the Rock concession stand that they have read the document in full. This plan is updated yearly.
